Transition of Young People into Adult Services Core Capabilities Framework

Introduction

The development of a Core Capabilities Framework for The Transition of Young People into Adult Services in England will build upon and be complementary to the previous and ongoing work undertaken by NHS England and Improvement (NHSE/I) and the Burdett National Transition Nursing Network. NHSE&I, in collaboration with key stakeholders are developing a National Framework for Transition. The Burdett Nursing Network are supporting healthcare professionals to develop best practice pathways and exemplar models of care. The aim of the work is to ensure that young people are actively prepared for moving into adult services and appropriately supported as a young person in this service, to ensure they remain engaged with their care and treatment to improve patient and family experience alongside improving long-term health outcomes.

The development of a Core Capabilities Framework will provide the health sector with a consistent approach to training for the successful transition of young people between children’s and adult services. Currently, there is no national framework that addresses this need.

Purpose and scope of the framework

The Core Capabilities Framework for the Transition of Young People will identify and describe the core skills and knowledge which the healthcare workforce needs in order to deliver high quality, compassionate, personalised and developmentally appropriate healthcare for young people transitioning from children’s to adult physical and mental health services. The aim is to enable greater consistency in training and assessment and will be applicable to both clinical and non-clinical staff across all levels of roles in the workforce.

Development of the framework

Skills for Health has been commissioned to provide project management and lead on the development of the framework. A steering group comprising key stakeholders including children and young people and their families and carers is guiding the development process.

The draft framework has now been developed and undergone a national consultation process and is undergoing a final approval phase as part of other related work.

It is hoped that the framework will be launched in 2023.
