One award winner

This award will be given to a group of people operating as a ‘team’ to deliver person-centred, integrated care across professional, organisational and/or sector boundaries. System-wide working is fundamental to the improvement of sustainable patient and service user care. Our health and care workforce are dependent on a collaborative approach which requires commitment and dedication from individuals working in complex care settings to improve patient experiences.

The winners of this award will demonstrate one or all the following through a process of integrated care:

  • Improvements in care/patient experience
  • A positive impact on the team
  • Service improvements

Assessment criteria

  • Clear understanding of the challenge, how it was resolved and what the impact was on the team, patient, service and/or finances
  • Underpinned by a robust process
  • Demonstrating keeping the patient at the centre of care
  • Improved patient outcomes or patient experience
  • Crosses professional, organisational and/or sector boundaries
  • A new way of working that has been fully embedded across traditional boundaries of working
  • An innovative and cost-effective solution
  • Evidence of more effective system working, e.g. reduced duplication of activities, reduced hand-offs, reduced number of different interventions required

Who can make the nomination?

Anyone can make a nomination, including members of the public.

  • Nomination process

    Nominators must complete the nomination form, which includes no more than 500 words about the nominee, demonstrating four of the criteria.

    The nomination process takes approximately 10-15 minutes. We would recommend reviewing the questions and returning with your responses. You can click ‘Save and continue later’ at any time during the nomination, in the top right-hand corner of the screen in the nomination form.

    As part of the nomination, we ask for some simple information about you, such as your name, job title and contact details. This is so we can let you know if your nominee is successful in winning an award, and so we can verify the nomination. We also ask for some information about your nominee, including their name, job title, contact details, an overview of what they do and why they are being nominated, and an answer to a few questions which relate to the award criteria shown in the bullet points above.
