The Lifetime Achievement Award will be given to an individual who can demonstrate achievement within healthcare over the course of their career.

The Assessment Criteria for the Lifetime Achievement Award:

  • Demonstrate substantial service in a healthcare setting over the course of their working life
  • Demonstrate a contribution to healthcare which is above and beyond their job requirements
  • Demonstrate the impact of their service has had, on individuals, communities and the wider healthcare sector
  • Demonstrate how the nominee has been a strong role model for the health sector/their organisation
  • Demonstrate being an effective colleague – being part of the team/supporting others, and how this excellence has improved standards in healthcare

Applicants must complete no more than 500 words in order to demonstrate the criteria.

Who can make the nomination?

Nominations can be made by managers, colleagues or service users/individuals and carers.
