Overview This course is designed to help practitioners manage the healthcare needs of people with long term conditions more effectively. The course is divided into four modules which introduce the Personalised Care Planning process and develops an understanding of associated skills and behaviours: The course is supported by a workbook.
- Module 1: An introduction to personalised care planning
- Module 2: Understanding how personalised care planning works
- Module 3: Skills and behaviours for effective personalised care planning
- Module 4: Case studies.
The course links with policy areas such as information prescriptions and self-care.

Assessment Modules are followed by a 20-minute assessment designed to test learners’ understanding of the personalised care planning process. Learning progress and test results can be tracked by managers for internal development and learners can keep a certificate of completion for CPD.
